What you can do with this hack is add multiple fields when someone creates a new thread. You can specify what fields can go in which forums.

To add, edit, or delete a custom field go into your ACP
Forums & Moderators --> Threads Field Manager

I had to reupload the files 2-3 times to get it to work on my site. Maybe try that?

well this is the first time I've tried to use the Releases add-on and when it's enabled, it's a blank page

I have it installed on 3.6.8...works great
